Comparative study of the dynamics of mental development of premature infants with intraventricular cerebral hemorrhage in their first year of life according to maternal assessment
Abstract
Relevance. The application of high medical Technology and advances in preterm infant care are keeping more and more newborns at risk for health and developmental problems alive. The aim of this work was to comparatively study the developmental dynamics of premature infants with different degrees of intraventricular cerebral hemorrhage (IVCH), which is one of the most severe conditions in the newborn period, at different variants of neurosurgical treatment of their complications in the first year of infants' life using maternal assessment of child development. Materials and Methods. The study included 77 children (40 boys and 37 girls) born at 28,3±3,1 weeks' gestation with diagnosed intraventricular hemorrhages. The children were divided into groups on the basis of treatment for posthemorrhagic hydrocephalus (PGH group) — 33 children (22 boys), hydrocephalus syndrome (HS group) — 26 children (13 boys); 18 children (5 boys) with 1-degree intraventricular hemorrhages without complicated variants of the course made up the comparison group (comparison g roup). The parental Ages and Stages Questionnaire, Third Edition (Ages and Stages Questionnaires-3, ASQ-3; Squires J. et al., 2009) was used to assess infant development at 3 and 12 adjusted months of age. Results and Conclusions. No differences were found between study groups of children with intraventricular hemorrhages in measures of communication skills, cognitive, and social development at 3 and 12 adjusted months of life. Motor development in large motor skills at 3 months of age was relatively higher compared to 12 months, p = 0,003 and p = 0,010 in the PGG and HS groups, respectively. At 12 months of age, coarse motor skills were lower in the PGG group than in the comparison group, p = 0,015 and for fine motor skills was higher in the comparison group than in the HS group, p = 0,032. A more pronounced influence of the degree of intracranial structural changes (degree of intraventricular hemorrhages) than of the type of treatment used (conservative or operative neurosurgery) on the developmental features of surviving infants was observed.
