ВВЕДЕНИЕ

Когнитивный подход к фобиям (Beck, Emery, 1985) акцентирует опосредующую роль паттернов мышления и ментальных образов, которые позволяют человеку адаптироваться к внешним и внутренним стимулам. Когнитивные процессы как таковые не рассматриваются в качестве причин тревоги или избегающего поведения, но представляют одну из детерминант – систему переработки информации, дисфункция которой может быть результатом взаимодействия врожденной биологической уязвимости и жизненной истории неудачного когнитивного научения. Терапевтическая редукция страха и избегания мыслится как результат изменения неадаптивных (как осознанных, так и бессознательных) мыслительных паттернов.

Условнорефлекторные и когнитивные модели фобий

Условнорефлекторная модель была предложена в конце 50-х годов для объяснения фобического поведения и выработки соответствующей терапии (Wolpe, 1975). Бихевиоральные принципы позволяют успешно справляться с фобиями с помощью процедур пролон­гированного погружения в стрессогенную среду in vivo или в воображении. Эта терапия известна под разными наименованиями; она используется в разных терапевтических программах (Marks, 1981). Рациональное обоснование процедуры погружения in vivo подразу­мевает привыкание к психофизиологическим реакциям, связанным со страхом, и постепенное угасание избегающего поведения. В такой пролонгированной и повторяющейся конфронтации с фобическим стимулом тревога пациента угасает, исчезают ее негативные последствия, в результате чего пациент «разучивается» проявлять фобическое избегание. Бергер и Макгауф (Berger & McGaugh, 1974) кри­тиковали бихевиоральную терапию за акцентирование периферических реакций и за использование понятий павловской теории и теории оперантного научения в качестве объяснительных принципов. Бергер и Макгауф переформулировали теории научения, сделав акцент на центральных когнитивных процессах переработки информации. Однако условнорефлекторная модель и когнитивные теории не являются взаимоисключающими: воздействие фобических стимулов в воображении или in vivo может активизировать латентные когнитивные схемы, хранимые в долговременной памяти и возникшие на основе прошлого опыта. Их активация может усиливать эмоциональное возбуждение и запускать избегающее поведение.
