Комплексное ПТСР: особенности психотерапии последствий пролонгированной травматизации

В статье рассматривается новая диагностическая категория — комплексное посттравматическое стрессовое расстройство (КПТСР), введенное в Международную классификацию болезней (МКБ-11). Анализируются различия в механизмах воздействия на человека ситуативной и пролонгированной травматизации. Описываются диагностические критерии КПТСР в сравне¬нии с ПТСР и ПРЛ (пограничным расстройством личности). Анализируется конструкт «нарушения Я-организации», объясняющий основной механизм воздействия комплексной травмы и включающий в себя нарушения регу¬ляции эмоций, самовосприятия и нарушения в отношениях. Показано, что разработанные для ПТСР психотерапевтические подходы недостаточны для терапии КПТСР. Рассматривается подход к психотерапии КПТСР в рамках консенсуса экспертов в области посттравматического стресса, обсуждаются его возможные ограничения. Описывается подход к терапии КПТСР, назван¬ный «компонентная психотерапия». Обсуждается специфика психотерапии комплексной травмы.
