Биполярное аффективное расстройство. Клинические аспекты и коморбидность с другими расстройствами

В обзоре литературы приведены особенности клинической картины биполярного аффективного расстройства (БАР). Подробно рассмотрены трудности диагностики БАР, обусловленные сложной клинической картиной и являющиеся причиной несвоевременной постановки диагноза и формирования неблагоприятного течения заболевания. Представлены данные о психических расстройствах, с которыми необходимо проводить дифференциальную диагностику БАР. Подробно описаны смешанные биполярные состояния, представляющие трудности в плане диагностики и терапии и являющиеся негативным прогностическим фактором, а также варианты течения БАР, в том числе и неблагоприятные формы. Рассмотрены данные об ухудшении когнитивных возможностей пациентов по мере увеличения числа эпизодов БАР, что может проявляться даже в период эутимии. Особое внимание уделено рассмотрению форм психической и соматической патологии, коморбидных БАР, ассоциированных со снижением качества жизни, неблагоприятным прогнозом, ростом инвалидизации, преждевременной смертности.

Клиническая картина БАР характеризуется разнообразием психопатологической симптоматики, типов течения заболевания. Особенности БАР имеют прогностическое значение, могут являться предикторами характера течения заболевания.
